• Proof of  Purchase/Residency (list of examples)
• Photo/Copy of a  Government Issued Photo ID

After we receive your email, you'll be sent a contract to e-sign.

Once you've signed the contract, you can call the office to pay the required Customer Deposit and/or Service Fee (606) 379-2211.

New customers will be charged a $75 Customer Deposit and a $25 Service Activation Fee.

Existing customers will be charged a $25 Service Activation Fee.
